The breast is composed of a skin envelope that surrounds glandular tissue and fat. The breast rests on a broad and thick skeletal muscle called the pectoralis. The shape of a particular breast is determined by the quality of the breast skin envelope, the tissue it is filled with, and the relationships of the breast to the surrounding anatomic landmarks, such as the axilla (armpit), sternum (breast bone), clavicle (collar bone) and abdominal wall. It is normal to have some asymmetry between the two breasts. When discussing natural breast asymmetry with our patients, we comment that 'a woman's breasts are sisters, not twins'. The constricted breast is characterized by both a small breast volume and a drooping appearance. In severe cases, this breast shape is referred to as a tuberous breast, or a tubular breast deformity. The constricted breast has a fold beneath the breast (the infra-mammary fold) that appears to be too high. The presence of a high infra-mammary fold constricts the lower portion of the breast. The upper portion of the breast sags over the lower portion of the breast. The base of the constricted breast is narrow.
